Recall Summary

Name of Product: Iristar Minoxidil Spray Bottles

Hazard: The recalled spray bottles contain minoxidil which must be in child-resistant packaging as required by the Poison Prevention Packaging Act. The packaging is not child-resistant, posing a risk of serious injury or death from poisoning if the contents are swallowed by young children.

 

Remedy - Free Replacement

Consumers will be receive a free replacement bottle that features compliant child-resistant packaging and improved labeling. This replacement will serve as the only corrective Remedy. 

Verification – Brief Disposal Video:
To verify that the original product has been safely discarded and to prevent any potential misuse, consumers will be asked to submit a short video (5–10 seconds) that includes:

- Opening the bottle and pouring out any remaining contents,
- A handwritten note displaying the consumer’s order number clearly visible in the frame, and
- The empty bottle being placed into a trash bin.

 This short video submission helps confirm the authenticity of the claim and ensures the remaining product has been properly disposed of. 


Consumers will also be requested to take a photo of the discarded empty bottle in the trash and upload both the video and photos via the Moralowen webform at https://octopus-gecko-8583.squarespace.com

The bottles and outer packaging have labeled “Iristar” and “Minoxidil Spray” on the front and ingredients and directions are found on the back.
